This role blends your clinical expertise with a holistic approach to care, addressing medical, behavioral, and social needs. What you'll need to have for success: - Minimum of 5 years as a Registered Nurse - Active RN license: compact, multi-state licensure - Strong background in care management as well as complex and chronic condition management - Skilled in motivational interviewing, patient education, and patient advocacy - Ability to work independently in a remote environment while staying connected to the team - Comfort with technology and multiple EMR systems What we would love for you to have: - Bachelor’s Degree (Registered Nurse) - Specialty Certification in Care Management (CCM) - Multilingual a plus The salary range for this role is between $78,000. 00 to $83,000. 00 per year.
